Anterior infarction refers to myocardial necrosis affecting the anterior wall of the left ventricle. This is most commonly due to occlusion of the Left Anterior Descending (LAD) artery —often called the "widow-maker" due to the large amount of myocardium at risk. Prompt ECG recognition is critical for time-sensitive reperfusion therapy. 1. The ECG Leads & Anatomical Correlation The anterior leads are V1 through V4 (sometimes extending to V5, V6, and I/aVL depending on the LAD occlusion site).
